Yes. By returning this Image Release form electronically, I hereby give the photographer permission to use images from this session of myself, dependent children and family for professional purposes such as online portfolios, websites, displays, contests, and advertising, to promote the photographer's name or business. I have the legal authority to grant these permissions and I accept all responsibility for such.

I agree to the following *

We will arrive on time. Session times will not be extended due to late arrival. If the photographer has time, the session may be extended for $50 per 15 minutes.

We will arrive hair & makeup ready, clothing will be steamed or pressed and nails manicured. *Makeup should be worn and slightly heavier than normal (but not stage makeup.. dancers know what we mean here!).

We will not place cell phones, wallets or other items in pockets and will remove hair ties/other items we do not want in the photos.

If any one in my household is sick or has recently been exposed to sickness or if the photographer is sick, the session will be rescheduled.

If the photo session is for someone 19 or under, I am the parent or guardian and I am signing this form.

If the photo session is for someone 19 or under, I understand a parent or guardian must attend the session.
